can help by providing you with the facts you need in order to correct the
situation once and for all. Although there are a number of specific steps we follow and techniques we use when conducting an independent investigation, our Investigations service is centered around five (5) simple steps:

1. Establish the Investigation Scope;

2. Conduct Interviews and Research;

3. Determine the Root Cause(s);

4. Formulate Practical Recommendations and implement Preventive Control Measures

5. Issue Investigation Report.

The end result of our investigation is an honest, straight-forward written report defining the original root cause(s) of the problem, the sequence of events that led up to it (in flowchart format) and a number of recommendations targeted towards:
• Correcting the existing situation (reactive); and
• Preventing its reoccurrence in the future (proactive)
